A University needed to issue a questionnaire to all its 750 staff members. They requested that against each question the respondant could enter additional comments, and these extra responses will be collated at the end of the process. An additional requirement was to allow the respondants to partially complete the questionnaire and be able to return and finish it later.

We developed an on-line questionnaire system, where each respondant could log on, identify their department and other characteristics and proceed with answering the questions. The system suported multiple choice, textual and numerical answer types. At the end of the questionnaire the repondants were able to review their answers and add any final comments.
